Formation of an ISPE Student Chapter

The three requirements for Student Chapter applications are listed below, and can all be submitted through our online application, including uploading the two letters.

1. Submission of a letter from the University or Educational Institution indicating that the ISPE Student Chapter has approval to form.
2. A letter from the future Chapter President and/or Faculty Advisor that includes;
   1. The chapter name, the Faculty Advisor's name, and the Chapter President's name
   2. A detailed description of the student's interest and capacity in forming an ISPE Student Chapter (i.e. Why do the students at your university want to form an ISPE Student Chapter?)
   3. An outline of how often the Student Chapter will meet and the types of pharmacoepidemiologic activities it plans to participate in
   4. An introduction to the Faculty Advisor, including an explanation of why this person is capable of serving for your organization and your advisor's involvement with ISPE and pharmacoepidemiology
3. Complete the online application and upload the letters above. You will need to submit:
   1. The name/email of who is submitting the application
   2. The name of the chapter, contact information for the faculty advisor, and chapter leadership
   3. Confirmation that the chapter meets ISPE membership requirements
